If you have scheduled an initial appointment with one of our therapists and would like to fill out the the identifying information and personal history sections of our intake forms prior to coming to the office, please click on one of the following links to open the appropriate form, print it from your computer, fill it out at home, and bring it with you to your appointment.  Please note that there will be some additional policy forms to read and sign when you come to the office, so you should still arrive early for your first appointment.
